ELECTRONIC HEALTH INFORMATION (EHI) EXPORT (§170.315(B)(10)

This document describes EproMedical Export capability. The exports allow users to create an XML file of an individual patient, or all patients within a clinic. This document describes the types of exports that are available, the format of the exports, and additional details that are specific to its structures.

EXPORT USE CASES

1. All patient data that belongs to a clinic. This export can be used to export all patient’s data of a clinic from one database. Due to the large amount of data the exports process may require a long period of time to run and the files are usually very large.  

2. Single patient export is useful to export one patient’s health information for sharing with other providers or to satisfy a patient request for his/her health information. This export is downloaded at the time of the request.

Note: All export data is considered Protected Health Information (PHI) and requires that all guidelines be used to ensure that the information is encrypted and stored appropriately after download.

EHI EXPORTS TYPE

EproMedical offers comprehensive and structured data in the C-CDA format.

The following list of patient documentation will be exported in this format:

1. Patient Demographics

2. Practice Information

3. Allergy, Adverse Reactions, Alerts

4. Medication List

5. Diagnosis / Problem

6. Vitals Signs

7. Assessment

8. Treatment Plan

9. Procedures

10. Chief Complain

11. Visit Notes

Note: The common method of documentation for healthcare providers is the SOAP note (subjective, objective, assessment, and plan). Subjective would include the Chief Complain, and the objective would include in the Visit notes.

EHI EXPORTS FILE The export will be a zip file that contains a directory with the name ccda. Under the directory there will be an individual C-CDA document for each patient. The naming convention for those documents is the Patient last name and first name followed by the file extension of .xml.
